This is what happens in debate when one side is crazy. Debate is
impossible across the divide of sanity. A large school of
(substantially misguided) clinicians treat psychological depression
and anxiety with rational argumentation. No one treats paranoia that
way. The therapist cannot ally with the faculty that is most
compromised. Or address political argument to it.

Fortunately, paranoia is often limited to the specific subjects of the
delusional system.
